
This Pediatric Home Care program provides coordinated, family-centered care for infants, children, and youth in the home and community. We partner with families, primary care providers, and hospital teams to support safe transitions home, manage ongoing health needs, and promote comfort, independence, and quality of life. Services are tailored to each child’s care plan and can be scheduled for short visits, extended shifts, or ongoing support.

Personal Support Services
Personalized assistance to children with their daily personal care, routines, and activities while promoting comfort, dignity, and independence.

Nursing Services
Our Pediatric Nursing Services provide specialized clinical and personal care tailored to each child's medical needs, delivered by trained and qualified nursing professionals.

Infusion, injections, medications, tube feedings, G-tube, ostomy, and catheter care

Wound care, health assessments, and ongoing monitoring of the child's condition

Respiratory care including suctioning, oxygen, BiPAP, CPAP, and chest physiotherapy

Seizure monitoring and management according to the child's care plan

Personal care, mobility support, and nursing accompaniment to approved appointments and activities


School Nursing
Our School Nursing Services provide one-on-one nursing support to help children safely manage their medical and personal care needs while participating in the school environment.
