$200/month

Once a month in-home nursing visits. Ideal for individuals with one or more underlying medical conditions. Including:

Comprehensive Health Assessment: Head-to-toe evaluation, including vital signs and all major body systems.
Environmental Safety Check: Identifying any potential hazards affecting mobility and independence.
Wellness & Cognitive Review: Assessing mental health, hygiene, nutrition, and cognition for early intervention.
Medication Review: Ensuring proper usage and effectiveness.
Patient & Family Education: Guidance on disease management, medications, lifestyle changes, and prevention.
Family Update Call: One call with family/POA after nursing visits.

2. Bimonthly Care Package

$350/month

Two in-home nursing visits per month. Ideal for individuals seeking more frequent support

All of what’s included in our Monthly Check In with more frequency
Regularly monitoring patients for potential health issues and providing early detection
2 update calls with family/Power of Attorney (POA) after Nursing visits

3. Weekly Care Package

$625/month

Weekly in-home nursing visits. Ideal for individuals seeking regular support

Weekly follow-up calls with family/POA after nursing visits
